Health inspections can feel unpredictable, but the violations that show up most frequently are well-documented and largely preventable. Understanding what inspectors look for most often — and why these issues are so dangerous — is the first step toward consistently passing inspections and protecting your customers.

1. Improper Temperature Control for TCS Foods
Temperature violations are the single most common critical finding in food safety inspections. TCS (Temperature Control for Safety) foods must be held at 41°F or below for cold holding, or 135°F or above for hot holding. The temperature danger zone between these two numbers — 41°F to 135°F — is where most foodborne illness pathogens thrive.
Common examples: Sliced deli meats left on a prep table above 41°F, soup held in a steam well below 135°F, salad bar items not properly chilled.
Fix it: Calibrate thermometers regularly and check temperatures every 2–4 hours. Log the results. Train all staff on temperature requirements for every food category.
2. Poor Personal Hygiene — Especially Handwashing
Improper handwashing is consistently among the top violations cited. The FDA Food Code requires employees to wash hands for at least 20 seconds with soap and warm water at specific moments — before starting work, after touching raw meat, after using the restroom, after coughing or sneezing, and after handling garbage, among others.
Common examples: Employees going from handling raw chicken to ready-to-eat foods without washing hands; bare-hand contact with ready-to-eat foods in states that prohibit it; no visible handwashing between tasks.
Fix it: Post handwashing reminder signs at every sink. Make handwashing an observable habit, not just a policy. Ensure sinks are always stocked with soap and paper towels.
3. Cross-Contamination Between Raw and Ready-to-Eat Foods
Cross-contamination occurs when harmful pathogens transfer from raw animal foods to ready-to-eat foods that won't be cooked further. This is a leading cause of foodborne illness outbreaks.
Common examples: Raw chicken stored above ready-to-eat salad greens in the walk-in cooler; using the same cutting board for raw beef and vegetables without sanitizing between uses; raw meat dripping onto produce in the cooler.
Fix it: Store raw proteins below and separated from ready-to-eat foods in refrigerators. Use color-coded cutting boards. Sanitize all surfaces between different food types.
4. Inadequate Cooling Procedures
Improper cooling is one of the most dangerous violations because it allows pathogens to multiply to dangerous levels in large batches of food. The FDA Food Code requires cooked TCS foods to cool from 135°F to 70°F within 2 hours, and from 70°F to 41°F within an additional 4 hours — for a total cooling time of no more than 6 hours.
Common examples: Large pots of soup or chili placed directly in the walk-in cooler while still hot; whole roasts or large cuts of meat cooled in one piece rather than divided into smaller portions.
Fix it: Use ice baths, blast chillers, or ice wands to cool food rapidly. Divide large batches into shallow pans no more than 2–4 inches deep. Log cooling times and temperatures.
5. No Certified Food Protection Manager On-Site
In the 40+ states that require a Certified Food Protection Manager, operating without one present during hours of operation is a critical violation. This is also the violation most likely to result in a failed inspection for establishments that are otherwise well-run.

Common examples: Chlorine bleach solution below 50 ppm or above 200 ppm; quaternary ammonium (quat) sanitizer outside the 200–400 ppm range; sanitizer buckets not changed frequently enough.
Fix it: Post sanitizer mixing instructions at every prep area. Use test strips before every use and whenever the solution is changed. Change sanitizer solutions every 2–4 hours or when visibly soiled.
7. Improper Thawing Methods
Thawing frozen food on a counter at room temperature allows the outer layers to enter the temperature danger zone while the interior is still frozen. The FDA Food Code permits only four approved thawing methods.
Approved methods: (1) In a refrigerator at 41°F or below; (2) Under cold running water (70°F or below); (3) In a microwave, if immediately cooked afterward; (4) As part of the cooking process.
Fix it: Post approved thawing methods in the kitchen. Plan ahead — large proteins may take 2–3 days to thaw safely in a refrigerator.
8. Pest Evidence
Evidence of rodents or insects is among the most serious violations an inspector can cite. Even a single rodent dropping can result in an immediate closure order in many jurisdictions. Pests are attracted by food debris, moisture, and entry points in the building envelope.
Fix it: Maintain a professional pest control contract with regular scheduled visits. Seal all gaps around pipes and entry points. Keep receiving areas clean and inspect incoming shipments. Store all food 6 inches off the floor.
9. Bare-Hand Contact With Ready-to-Eat Foods
Most states prohibit bare-hand contact with ready-to-eat foods — foods that will not be cooked before being served. This includes salads, sandwiches, garnishes, bread, and cooked items being plated. Bare hands harbor pathogens even after handwashing.
Fix it: Use gloves, tongs, deli tissue, or utensils for all ready-to-eat food handling. Note that gloves are not a substitute for handwashing — hands must be washed before putting on gloves.
10. Date Labeling Violations
Ready-to-eat TCS foods prepared on-site must be date-labeled and discarded within 7 days (or sooner, per your establishment's HACCP plan). Opened commercially packaged TCS foods must also be date-labeled once opened. Unlabeled or expired foods are a consistent inspection finding.
